- for reporting hours worked by individual Scouts on activities that were not organized by
the Troop |
 | Troop Activity Form - to be used by the
leader/coordinator of a Troop-organized campout, day outing, or service project to
document the number of nights camped, miles biked/hiked, or service hours of multiple
Scouts |
Treasury Forms
 | Scout Account Spending Form - use this to
make a withdrawal from a scout account to pay for outing or activity fees, dues, summer
camp, etc. |
 | Reimbursement Request Form - use
this to request reimbursement for troop expenses. The detailed reporting will help
us track where we are spending our money. |

Health Forms (forms last updated from Council web

 | Health and Medical Record - Class 3
 | For adults over 40 years of age when any physical examination is required and for all
participants of high-adventure activities within the Greater St. Louis Area Council. |
 | Note: Philmont and other high-adventure bases outside of our Council may require the use
of a different form. |
 | Must be signed by a physician no more than 1 year prior to the activity. |

 | Health and Medical Record - Class 2
 | Only for youth or for adults under 40. |
 | Necessary for summer camp and other activities lasting longer than 72 hours. |
 | Page 2 must be completed by a physician. |
 | Must be completed every 3 years for Scouts and for adults under 40 years of age. |

 | Health History - Class 1
 | Used for annually updating the Class 2 Health and Medical Record for Scouts and adults
under 40 years of age. |
 | Valid for one year only--does not require a physician's signature. |
