Advancement Committee Forms

bulletIndividual Record of Service - for reporting hours worked by individual Scouts on activities that were not organized by the Troop
bulletTroop Activity Form - to be used by the leader/coordinator of a Troop-organized campout, day outing, or service project to document the number of nights camped, miles biked/hiked, or service hours of multiple Scouts

Treasury Forms

bulletScout Account Spending Form - use this to make a withdrawal from a scout account to pay for outing or activity fees, dues, summer camp, etc.
bulletReimbursement Request Form - use this to request reimbursement for troop expenses.  The detailed reporting will help us track where we are spending our money.

Leadership Contract and Information

bulletLeadership Contract must be signed by each Scout running for election to a Troop or Patrol leadership position.  It must also be signed by the Scout's parent and Scoutmaster.
bulletTroop and Patrol Leadership Responsibilities describes each elected position in the Troop.

Health Forms (forms last updated from Council web site on October 9, 2006)

bulletHealth and Medical Record - Class 3
bulletFor adults over 40 years of age when any physical examination is required and for all participants of high-adventure activities within the Greater St. Louis Area Council.
bulletNote: Philmont and other high-adventure bases outside of our Council may require the use of a different form.
bulletMust be signed by a physician no more than 1 year prior to the activity.
bulletHealth and Medical Record - Class 2
bulletOnly for youth or for adults under 40.
bulletNecessary for summer camp and other activities lasting longer than 72 hours.
bulletPage 2 must be completed by a physician.
bulletMust be completed every 3 years for Scouts and for adults under 40 years of age.
bulletHealth History - Class 1
bulletUsed for annually updating the Class 2 Health and Medical Record for Scouts and adults under 40 years of age.
bulletValid for one year only--does not require a physician's signature.
